In past years, the standard approach that nearly all Trans people took once completing their transition was to, "go stealth"; Cut all ties w/ everyone they knew, which was tragically pretty easy as most of the time their friends and family would disown them :( and move to an entirely different city far away and start a new life. There are many older Trans adults out there today, just living in the background, and most Cis people would never suspect they are are different from anyone else. Today many of us are choosing to live openly instead of trying to erase our past.
